Team AmeriFace Terms of Participation
All Team AmeriFace members (athletes and non-athletes) are required to, and hereby do, assume all risk of participation in any and all events by signing this general release agreement.
By completing and submitting a Team AmeriFace enrollment form, either by electronic or written means, you agree to the following Terms of Participation. You are required to review this document periodically to check for updates to its content.
Contact with Team AmeriFace and AmeriFace-Related Programs
All Team AmeriFace members will be added to the organization-wide general contact and distribution list. Contact the AmeriFace administrative office with changes to your contact information or to opt-out of future correspondence.

Code of Ethics
All persons associated with AmeriFace and related programs and activities are expected to honor our Code of Ethics.
Events
Team AmeriFace will work to raise awareness, promote the mission and raise funds to support AmeriFace programs. Team AmeriFace will field teams of athletes and volunteers at various events nationally and globally. A Team may consist of one or more athletes and/or volunteers, each displaying the Team AmeriFace logo on a bib, t-shirt, cap or with a banner, sign or other promotional instrument.
Team AmeriFace members will honor the missions of other organizations and their events. Therefore, Team AmeriFacewill not field a team for events designed to support specific organizations and/or causes/conditions; in other words, Team AmeriFace will assemble teams only for "generic" events.
Fundraising
Team AmeriFace is set up to accept donations and pledges through various means. Team AmeriFace members are instructed to set up an Official Team AmeriFace Cause Page at Facebook, powered by Network for Good. Step-by-step instructions are posted at Team AmeriFace website on the "Join" page.
Supporters may choose to donate directly to Team AmeriFace through the "Donate" page at the Team AmeriFace website. They may also choose to donate by mail. Checks should be made payable to AmeriFace and sent to our administrative office address, posted on the "Contact" page at all AmeriFace-related websites.
Every donor, if they submit adequate contact information, will be issued a receipt for their tax-deductible donation. AmeriFace is an IRS tax-exempt public charity (#32-0085490). Donors may specify the program they wish to support when they make their donation.
Team AmeriFace members are under no obligation to participate in fundraising activities; however, Team AmeriFace members are expected to wear an official Team AmeriFace bib to promote awareness for the cause.
Referrals
Team AmeriFace members agree to forward all patient/family inquiries to the AmeriFace administrative offices for appropriate referral. See the Contact page for additional information.

Release of Liability
As a member of Team AmeriFace, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless AmerFacee, Team AmeriFace, all AmeriFace-affiliated programs and events, their officers, officials, employees, volunteers, agents, independent contractors, including official event videographers(s) and/or photographer(s), whether contracted or volunteer in nature, any sponsor(s) and/or presenter(s) of any portion of any event, or affiliates of these entities, and, if applicable, owners and lessors of any premises used for Team AmeriFace activities, and the organizers and/or sponsors of various events in which Team AmeriFace members may participate (“Releasees”), with respect to any and all injury, illness, disability, death or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.
I represent and warrant that I have consulted my physician and have no condition which would prevent me from engaging in Team AmeriFace activities. I agree that I am solely responsible for assessing the risk associated with participating in these activities.
If the participant is a minor child, I do hereby give permission for said minor child to participate in the events and agree that I, or a competent adult designated by me, will be accessible at all times while minor child is engaged in the events.
I further represent and warrant that the adult(s) in charge of said minor child shall be solely responsible for assessing the risk to said minor child and determining in which activities said child is allowed to engage.
I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation and/or the participation of said minor child.
Videography/Photography Permission and Release
I hereby give AmeriFace, any and all entities under the corporate structure of AmeriFace, any official event photographer(s) and/or videographer(s), whether contracted or volunteer in nature, any sponsor(s) of any portion of events which include the participation of members of Team AmeriFace, print and broadcast media outlets, or affiliates of these companies, the absolute and irrevocable right and permission, with respect to the photographs, video and/or film that are being taken of me and/or my minor child(ren) or in which we may be included with others, or photographs that were submitted by me for consideration for publication and/or other use:
a. To copyright the same in their own name or any other name that they may choose;
b. To use, reuse, publish and republish the same in whole or in part, individually or in conjunction with other photographs, in any medium and for any purpose whatsoever, including (but not by way of limitation) illustration, promotion and advertising and trade; and,
c. To use my/our name(s) in connection therewith if they so choose.
I hereby release and discharge AmeriFace, including any and all entities under the corporate structure of AmeriFace, including Team AmeriFace, any official event photographer(s) and/or videographer(s), whether contracted or volunteer in nature, any sponsor(s) of any portion of events which include the participation of members of Team AmeriFace, print and broadcast media outlets, or affiliates of these companies, from any and all claims and demands arising out of or in connection with the use of the photographs, including any and all claims for libel.
This authorization and release shall also endure to the benefit of the legal representatives, licensees and assigns of the above-named entities as well as the person(s) for whom the photographs are taken.
I grant permission for the publication of photographs as described above and will in no way hold AmeriFace or its agents responsible for unauthorized use of said materials.
I HAVE READ THIS TERMS OF PARTICIPATION DOCUMENT, INCLUDING THE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, I FULLY UNDERSTAND ITS TERMS AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
I understand that by clicking the "JOIN!" button on the Team AmeriFace "Enroll" page, I am submitting my electronic signature on this agreement.
